- Issues Affidavits ($34.00 plus HST for the first document. $7.50 plus HST for each additional document)
- Issues Declarations ($34.00 plus HST for the first document. $7.50 plus HST for each additional document)
The Office of the City Clerk does not issue Certified True Copies or provide the services of a Notary Public.
The following is a list of documents that will be commissioned at the City:
- Planning Applications
- Legal Name Change Application
- Name Change and/or Sex Change documents and applications
- Declaration Affirming Parentage
- Election to Change Name of Child under the age of 12 (Service Ontario form)
- Vehicle Ownership Transfers — Sworn statement for a transfer of a used vehicle in the Province of Ontario
- Pension Purposes — Attestations a person is alive and confirming residency
- Delayed birth registrations
- Lost passport
- Common law or single status affidavits
- Statement of Conscience of Religious Belief — Immunization of School Pupils Act
- Municipal planning applications related to matters within the geographical limits of the Municipality
- Consent letters for children travelling out of the country
- Construction Act documents for progress and/or completion
Please ensure you have the following:
- The document requiring a Commission of Oath signature. The document should be completed but do not sign until you are before the Commissioner.
- One valid piece of government issued photo identification. Photocopies will not be accepted.
